The day in summary

The 25 April 2026 Primary Care Masterclass was the first in-person Acumen Academy event. The format combined founder-led teaching with case-discussion segments, small-group breakouts, and structured Q&A. Delegates received a CPD certificate suitable for the GP appraisal portfolio.

The day was structured around the Diabetes-Cardiorenal-Metabolic (DCRM) 2.0 framework as it applies to UK primary care, with particular focus on how the SGLT2-inhibitor, GLP-1 receptor agonist, and finerenone evidence bases have reshaped what UK primary care is now expected to deliver.

Session 1 · The DCRM 2.0 framework in UK primary care

Why diabetes, cardiology, and renal medicine can no longer be managed in silos. The DCRM 2.0 multispecialty practice recommendations and their UK primary care application. Designing a community DCRM clinical pathway.

Session 2 · SGLT2 inhibitors and GLP-1 receptor agonists

Clinical positioning, evidence base, prescribing decisions in primary care. Combination therapy. Real-world UK primary care prescribing patterns and lessons from current cohort data.

Session 3 · The cardiorenal continuum

CKD with proteinuria identification and management in primary care. Finerenone and the cardiorenal pathway. Heart failure with preserved and reduced ejection fraction. Quadruple therapy in primary care.

Session 4 · Lipid optimisation and ASCVD prevention

ApoB and Lp(a) screening: when, why, what to do with the result. Statin optimisation. Beyond statins — bempedoic acid, PCSK9 inhibitors, oral agents. Familial hypercholesterolaemia identification.

Delivered by

The masterclass was led by Professor Rajesh Varma — Professor of Primary Care, NICE Guideline Committee member, Visiting Researcher at the University of Cambridge Department of Public Health and Primary Care, and a practising GP Partner and GP Trainer in Hertfordshire — with case-based discussion supported by Professor Tham Nimal-Raj, Co-Founder of MD Acumen and Chancellor of New Vision University UK Campus.

What attendees took away

The cohort included GPs, pharmacist independent prescribers, and nurse practitioners practising across Hertfordshire and surrounding regions. Feedback supported the integrated cluster approach and the use of real-world cohort cases. The format and curriculum architecture from this masterclass have informed the structure of the forthcoming Acumen PG Diploma in DCRM (planned Q1 2027 launch, subject to accreditation timing).
