01 · Context

The UK pathway in 2026

The UK remains one of the most internationally consequential clinical training environments in the world, with a long-established National Health Service, a postgraduate training architecture under the Medical Education England framework, and a Royal College system that produces credentialled specialists whose qualifications are recognised globally. For international medical graduates, the UK is also one of the most pathway-complex destinations: registration depends on the General Medical Council, postgraduate training entry depends on NHS England (or the devolved equivalents in Scotland, Wales and Northern Ireland), and language and visa requirements depend on the Home Office and on UK Visas and Immigration.

Two structural changes are reshaping this pathway in 2026. First, the NHS Long Term Workforce Plan (published 2023, currently in implementation) is expanding domestic medical school numbers, which over time will shift the supply-demand balance for IMG entry into specialty training. Second, the GMC has announced a phased transition from the Professional and Linguistic Assessments Board (PLAB) examination to the UK Medical Licensing Assessment (UKMLA), which from 2024 onward also became the licensing examination for UK medical school graduates. During the transitional period, both examinations may co-exist for different cohorts; candidates should consult the GMC website at the date of application for the examination route applicable to them.

02 · Two routes

Two pathways into UK clinical practice

Most international medical graduates seeking UK practice will follow one of two pathways. Each pathway has different eligibility criteria, different timelines, and different downstream consequences for entry into specialty training.

Pathway A · UK Foundation Programme

UKFP entry, or F2 Standalone for those with one prior internship year

The traditional structured pathway, with national application via the Eligibility Office of the UK Foundation Programme Office (UKFPO).

03 · Pathway A

UK Foundation Programme — UKFP entry and F2 Standalone

The UK Foundation Programme is a two-year postgraduate training programme that all UK medical school graduates enter, and that international medical graduates can enter through the UKFP Eligibility Office route. Foundation Year 1 (F1) leads to full GMC registration on satisfactory completion; Foundation Year 2 (F2) leads to the Foundation Programme Certificate of Completion (FPCC), which is the gateway to most UK specialty training applications.

Two sub-routes apply to international medical graduates:

A1

UKFP Eligibility Office route

For graduates of accepted overseas medical schools who have not undertaken a postgraduate clinical year abroad. Application via the UKFPO Eligibility Office; if eligible, candidates compete in the national UKFP application alongside UK medical school finalists. Entry is by the Educational Performance Measure (EPM), the Situational Judgement Test (SJT) and any local additions; from 2025 onward the EPM has been replaced by the Multi-Specialty Recruitment Assessment (MSRA) for several entry cohorts and applicants should check the UKFPO website for the assessment configuration applicable to their year.

A2

F2 Standalone route

For graduates who have completed a one-year postgraduate clinical internship in their country of qualification — typically equivalent to UK F1 — and who therefore enter at F2 directly. F2 Standalone posts are recruited regionally rather than through the central national application; availability and competition vary by region and year. Successful F2 Standalone completion yields the FPCC and the same downstream specialty training entry options as a full UKFP completion.

Eligibility documentation for both UKFP routes is administered by the UKFP Eligibility Office and typically includes: medical school transcript and certificate; primary source verification via the GMC's accepted verification service (currently MyIntealth, formerly EPIC); evidence of English-language proficiency to GMC standards (IELTS or OET, at the levels published by the GMC); satisfactory references; and confirmation of immigration status. Applicants should consult the UKFP Office's published eligibility guidance at the date of application.

03b · Parallel timetable

The Pathway A timetable in calendar order — GMC, PLAB and UKFP tracks side by side

The UKFP Eligibility Office route requires three regulatory processes to run in parallel: GMC provisional registration (with PLAB 1 and PLAB 2 as gateway examinations for IMGs entering via the international route), the UKFP eligibility application on Oriel, and the UKFP application for Foundation School allocation. The timetable below sets out the calendar-ordered milestones for an applicant entering Foundation Year 1 on 5 August 2027, with the GMC, PLAB and UKFP processes mapped against each other and against the statutory prioritisation framework introduced by the Medical Training (Prioritisation) Act 2026.

Important — projected dates for the UKFP 2027 cycle. At the time of editorial review, the UK Foundation Programme Office has published the full UKFP 2026 Eligibility Applications Timeline and Deadlines. The UKFP 2027 timetable has not yet been formally released. All dates marked with an asterisk (*) in the table below are projected estimates for the UKFP 2027 cycle, derived by mapping the UKFP 2026 timetable forward by twelve months. They are indicative only. Applicants must confirm every date against the UKFPO's official UKFP 2027 publication once issued.

Step 0 · Foundational verification (pre-application, throughout 2026)

Before any application window opens, three foundational workstreams must be initiated. All three have lead times of several weeks to several months and should be started early in 2026.

Primary source verification of the PMQ via ECFMG / Intealth

The GMC requires primary source verification of your primary medical qualification through ECFMG / Intealth online services. This verification is also recognised by international regulators in the United States, Australia and elsewhere, and so opens both the UK and international internship markets to you post-MD. See the GMC's guidance on primary source verification for international medical graduates and the list of which qualifications need to be verified.

English-language evidence — IELTS, OET, or PMQ-as-evidence

Sit IELTS (Academic) via the British Council or OET (Medicine) via the Occupational English Test to GMC standards, or — if eligible — rely on your PMQ as evidence per the GMC's published list of countries where English is the first and native language. Plan the test date so the certificate remains within validity at the point of GMC registration application (typically mid-2027).

PMQ acceptability — UKFP PMQ confirmation form and (if needed) the GMC PMQ1 query form

If your PMQ certificate has not yet been issued (because you have very recently qualified, or are still a medical student), you must submit the completed UKFP 2026 PMQ confirmation form (.docx, direct download), signed and stamped by the medical school Dean or Registrar's office. Where the medical school is not clearly on the GMC's accepted list, an early enquiry via the GMC PMQ1 Primary Medical Qualification Query Form (.docx, direct download) is also advisable — you must complete the form yourself, attach every applicable evidentiary document listed below, and return it to the GMC.

Documents required by the GMC PMQ1 Primary Medical Qualification Query Form

The GMC PMQ1 query form will not be progressed unless every applicable document below is submitted. Complete and accurate English translations are required for every document not originally in English. The form requires that you complete it yourself; any part completed by a third party will be rejected.

  1. Primary medical qualification degree certificate — copy, with English translation where appropriate.
  2. Full transcripts for each university where you have undertaken medical studies, with English translations where appropriate.
  3. Evidence that your qualification allows you to register to practise medicine in the country that awarded your qualification — this could include evidence that you are eligible to undertake a period of postgraduate clinical training.
  4. Letter from your awarding body confirming course length — specifying (a) the standard length of the course in years, (b) the standard length of the course in clock hours (not credit hours), (c) the minimum number of hours a student must complete before being eligible for graduation, and (d) the number of years and clock hours that you personally completed of that course. The GMC defines a clock hour as time spent at medical school in lectures and completing laboratory work; it includes time spent on practical work as part of clinical rotations and, where applicable, your pre-graduate internship — but excludes self-directed study and any semester, exam or year that has been repeated.
  5. Letter from your awarding body confirming clinical rotations undertaken prior to any pre-graduate internship — specifying (a) the dates of your clinical rotations, (b) the number of clock hours completed in each clinical rotation, (c) the location of each clinical rotation, (d) that the awarding body arranged, oversaw and approved the clinical rotations, (e) that the awarding body holds written agreements with the clinical rotation providers, (f) that the clinical rotations included exposure to medicine and surgery, and (g) that the clinical rotations increased in duration, in-person clinical interaction and responsibility as the course progressed.
  6. Letter from your awarding body confirming pre-graduate internship (if applicable) — specifying (a) the dates of your pre-graduate internship, (b) the rotations completed, (c) the number of hours completed in each rotation, (d) the location and country of the institutions where you completed your pre-graduate internship, (e) that the awarding body arranged, oversaw and approved the pre-graduate internship, and (f) that the awarding body holds written agreements with the internship provider. Do not provide post-graduate internship evidence — the GMC does not consider it at this stage.
  7. Letter from your awarding body confirming online or remote study — specifying whether any part of the course was completed online, remotely or not in person. If any part of the course was so completed, the same letter must additionally specify (a) the modules that were completed online, remotely or not in person, (b) which of those modules were theoretical or clinical/practical, (c) the dates that the modules were so completed, (d) the number of hours of study so completed, and (e) the mechanisms put in place during the online, remote or not-in-person study to check students' progress and understanding.
  8. Evidence from your awarding body confirming transferred credits (if you studied at more than one institution) — specifying (a) which credits the awarding body accepted from each previous university, (b) whether the awarding body accepted any credits from failed modules, and (c) the reason for transfer.
  9. Letters from each previous institution where you studied for your PMQ — specifying (a) the reasons for transfer, (b) whether or not you failed any part of the course or were awarded an alternative qualification, and (c) whether you would have been allowed to continue on the course had you not chosen to transfer.

Early July 2026* If PLAB 1 has not yet been booked, log in to your GMC Online account to view and book available PLAB 1 places. PLAB 1 must be booked on a UKFP-compliant date (see November 2026 row below). UKFP 2027 eligibility application window opens* (projected: approximately 1–22 July 2026, midday BST deadline*). Complete and submit the eligibility application form on Oriel. Confirm your GMC status, your booked PLAB 1 date, and your PMQ status — or upload the completed UKFP PMQ confirmation form. The eligibility application is the gateway. Eligibility outcomes are typically released approximately three weeks after the deadline.*
Late July – August 2026* Deadline to request priority access to UKFP-compliant PLAB 1 places with the GMC — projected late August 2026.* Priority access is only needed if you have already searched and cannot find a compliant date. Eligibility outcomes released on Oriel — projected mid-August 2026.* Latest date to qualify from medical school if you are required to undertake PLAB — projected end of August 2026.* If you are deemed ineligible at this stage, the UKFP route is closed for the 2027 cycle and you should pivot to Pathway B (Non-UK Internship → CREST).
September – October 2026* Continue PLAB 1 preparation. Confirm your sitting date is on or before the latest UKFP-compliant PLAB 1 exam date (projected early November 2026*). UKFP 2027 Foundation Programme (FP) and FPP application window* — projected approximately 23 September to 7 October 2026, midday BST deadline.* If you were deemed eligible or eligible-with-conditions, you must complete the separate FP application on Oriel, alongside any Pre-allocation application if applicable. This is the substantive UKFP application — distinct from the eligibility application. It feeds into national ranking and Foundation School allocation.
November 2026* Latest UKFP-compliant PLAB 1 exam date — projected early November 2026.* If you sit PLAB 1 after this date, you will not meet the deadline to send results to the UKFPO and your eligibility application will be withdrawn. Clinical Assessment test dates — projected early November 2026* — if you are required to undertake the Clinical Assessment. Outcomes issued approximately late November 2026.* The PLAB 1 deadline is the single most common point of failure for IMG applicants. Book early.
December 2026* PLAB 1 results published. Deadline to provide the UKFPO with evidence of PLAB 1 pass — projected approximately 22 December 2026, midday GMT.* Evidence to be sent to england.helpdesk.foundationprogramme@nhs.net, quoting the Oriel PIN associated with your application. Failure to meet this deadline results in withdrawal of the eligibility application.
January 2027* Deadline to request priority access to UKFP-compliant PLAB 2 places with the GMC — projected approximately 15 January 2027.* Priority access is only needed if you cannot find a compliant date through standard booking. National ranking and allocation processing on Oriel. PLAB 2 is the OSCE-style clinical and professional skills assessment.
February – April 2027* Sit PLAB 2 on or before the latest UKFP-compliant date (projected early May 2027*). Provisional allocation and Foundation School matching — projected from February 2027 onwards.* Priority-group allocation precedes the reserve list (see notes column). Medical Training (Prioritisation) Act 2026 — statutory effect. Priority-group applicants (UK medical school graduates and others within the statutory priority category) are allocated to Foundation Programme posts first. IMGs in the non-priority group enter a national reserve list and are allocated against unfilled vacancies arising from priority-group withdrawals. Per the UKFPO, reserve-list allocation via Preference Informed Allocation is projected to commence week beginning 15 June 2026 for the 2026 cycle, and is expected to follow a comparable pattern in 2027.*
May 2027* Latest UKFP-compliant PLAB 2 exam date — projected approximately 1 May 2027.* Throughout May 2027 apply for GMC provisional registration. You may apply up to three months before your F1 start date. Begin pre-employment processes with your allocated Foundation School and employing Trust, subject to allocation status (priority group or reserve list). The GMC's provisional registration evidence requirements include the PMQ, English-language evidence, passport, certificate of good standing, five-year activity history, fitness-to-practise declaration, identity check, and translations of any non-English documents.
June 2027* Deadline to submit your application for GMC registration — projected approximately 12 June 2027.* Deadline to provide the UKFPO with evidence of PLAB 2 pass (or entry onto the GMC register) — projected approximately 8 June 2027, midday BST.* Reserve-list allocation activity for non-priority IMG applicants commences, subject to vacancies arising from priority-group withdrawals — projected from week commencing approximately 14 June 2027.* Allocation may continue into July. Have your GMC application evidence ready ahead of time. Do not wait until PLAB 2 results are published to begin preparing the GMC application.
July 2027* GMC provisional registration granted (subject to satisfactory application). Final reserve-list allocations may continue into July 2027.* Mandatory induction and shadowing period typically begins in late July, with dates communicated by the employing Trust. You are advised to have GMC registration in place before the start of induction and shadowing.
4 August 2027* All applicants must have qualified from medical school by this date.* Final eligibility gate.
5 August 2027* You must hold provisional GMC registration by this date.* Foundation Programme start date — 5 August 2027.* Commencement of the two-year UKFP post. F1 leads to full GMC registration on satisfactory completion. F2 leads to the Foundation Programme Certificate of Completion (FPCC), the gateway to most UK specialty training applications.

Statutory context — the Medical Training (Prioritisation) Act 2026 and the Reserve List

UKFPO statement on the Reserve List (current text, applied to the 2026 cycle and expected to apply in materially similar form to the 2027 cycle):

Following the enactment of the Medical Training (Prioritisation) Act, the UKFPO is committed to allocating all priority-group applicants to foundation programmes first. When subsequent vacancies occur through applicant withdrawals, applicants from the national reserve list — made up of non-priority group applicants — will be allocated on Oriel via Preference Informed Allocation, using their computer-generated rank and their foundation school preferences. For the 2026 cycle the UKFPO indicated this process would commence week commencing 15 June 2026 and continue on a regular basis, with withdrawals and offers possibly extending into July. A proposed timetable and supporting guidance is published by the UKFPO in due course for each cycle.

Non-UK Internship → GMC registration → Trust-Grade FY2 → CREST

For international medical graduates who have already completed a non-UK clinical internship that the GMC accepts as equivalent to UK F1, the second pathway proceeds through the GMC's licensing examination route, into a clinical post in the NHS at FY2-equivalent level, and through the Certificate of Readiness to Enter Specialty Training (CREST).

GMC regulatory examination — PLAB or UKMLA

Sit the GMC's licensing examination route applicable to your cohort. PLAB has historically operated as a two-part examination (PLAB 1 written, PLAB 2 OSCE-style clinical and professional skills assessment); the UKMLA is the GMC's successor licensing examination, also delivered in two parts (Applied Knowledge Test and Clinical and Professional Skills Assessment). The GMC website is the authoritative source for which examination applies to your application year. MD Acumen does not provide PLAB or UKMLA preparation; candidates should consult the GMC's own resources and recognised examination preparation providers.

B2

Full GMC registration with a licence to practise

Following successful completion of the GMC's licensing examination, English-language evidence to GMC standards, satisfactory primary source verification of medical qualifications, and a satisfactory non-UK internship year, the GMC grants full registration with a licence to practise. This is the pre-requisite for substantive NHS clinical employment.

Trust-Grade FY2-equivalent post

Secure a clinical post within an NHS trust at a level equivalent to UK F2. These posts are recruited by individual trusts, not via the national UKFP application; titles vary (Senior House Officer, Trust Grade Doctor, Resident Doctor) and competition varies by region and specialty. The post is the working environment within which the CREST competences are evidenced.

CREST — Certificate of Readiness to Enter Specialty Training

The CREST is the structured competence framework that confirms an IMG who has not completed UKFP has achieved the equivalent of UK Foundation Programme outcomes. It is signed off by a clinical supervisor over the course of a Trust-Grade FY2-equivalent post. With CREST and Full GMC registration, applicants are eligible to apply to UK specialty training in the same recruitment rounds as UKFP completers.

05 · Eligibility

What the GMC accepts as a non-UK internship

The GMC publishes a list of countries whose internship years are accepted as equivalent to UK F1 for the purposes of full registration. The criteria broadly require that the internship was a structured supervised clinical year of at least twelve months' duration, undertaken after completion of the medical degree, in posts that included a balance of medicine and surgery (and sometimes other specialties depending on the country's framework), with formal assessment.

The list of accepted countries and the specific durations and configurations required is updated periodically. Applicants should consult the GMC's "Acceptable patterns of internship" page at the date of application. Non-UK internship years that do not appear on the accepted list will not satisfy the GMC's full registration requirements; candidates in this position will need to enter via Pathway A (UKFP, including F1 if a recognised non-UK F1-equivalent year was not completed).

06 · Comparison

Pathway A versus Pathway B — head-to-head

Pathway A · UKFP / F2 Standalone

Strengths. Structured national pathway; integrated with UK specialty training application; fewer separate steps; GMC registration follows F1 completion automatically.

Constraints. National competition; eligibility limited by the UKFPO criteria; F2 Standalone competition varies by region and year; cohort size constrained by national workforce planning.

Best fit. Recent graduates of accepted overseas medical schools; graduates with one structured postgraduate clinical year that satisfies F2 Standalone eligibility.

Pathway B · Non-UK Internship → CREST

Strengths. Available year-round (not tied to a single national application window); regional flexibility in Trust-Grade post selection; opportunities to gain UK clinical exposure before specialty training application.

Constraints. Multi-step pathway; PLAB or UKMLA examination required; Trust-Grade post must be secured independently; CREST competences must be evidenced and signed off; longer overall timeline.

Best fit. IMGs with completed GMC-accepted non-UK internships who are not eligible for or do not wish to apply to UKFP; candidates seeking flexibility on entry timing and region.

07 · Pragmatic recommendations

Five practical points for IMGs in 2026

  • Confirm GMC examination route at the time of application. The PLAB-to-UKMLA transition is in progress; the examination applicable to your cohort is determined by the GMC at the date of application, not at the date of decision. Do not rely on second-hand summaries; consult the GMC website directly.
  • Verify that your overseas internship is on the GMC's accepted list before committing to Pathway B. If it is not, the entire Pathway B route may be blocked and a UKFP-equivalent application may be necessary.
  • Plan English-language evidence early. IELTS and OET have validity periods. Sitting the test too early can mean expiry before a registration application is complete; sitting too late can delay GMC registration.
  • For Pathway B, build UK clinical exposure before applying for Trust-Grade posts. A documented UK observership — such as the MD Acumen Primary Care Observership — strengthens the application file and demonstrates orientation to NHS systems, professional standards, and the UK clinical environment.
  • Plan the visa framework alongside the clinical pathway. The Skilled Worker (formerly Tier 2) visa route is the principal pathway for non-UK IMGs entering NHS clinical employment. Visa eligibility, sponsorship requirements, and Health and Care Worker visa rules are administered by UK Visas and Immigration; clinical and immigration pathways must be planned together.

Honest answers to frequent questions

Does MD Acumen prepare candidates for PLAB?

No. MD Acumen does not run a PLAB academy and does not provide preparation for PLAB or the UKMLA. PLAB and the UKMLA are GMC-administered regulatory examinations; candidates should consult the GMC's own resources and recognised examination preparation providers.

Is PLAB still being offered in 2026, or has it been replaced by the UKMLA?

The transition from PLAB to UKMLA is a phased process administered by the GMC. The applicable examination depends on the cohort and the application year. Candidates should consult the GMC website at the date of application for definitive guidance.

How long does the full Pathway B route typically take?

From completion of a non-UK internship through PLAB or UKMLA, GMC registration, securing a Trust-Grade FY2-equivalent post, and CREST sign-off, the realistic timeline is twelve to twenty-four months, depending on examination scheduling, post availability, and the pace of CREST competence acquisition. UKFP applicants by contrast follow the national application calendar, which is published annually.

Can I apply to UK specialty training without UKFP and without CREST?

The standard requirement for entry to most UK specialty training programmes is either FPCC (UKFP completion) or CREST. Limited specialty-specific exceptions may apply for certain candidates with extensive equivalent international training; applicants should consult the relevant Royal College and NHS England specialty recruitment guidance.

Does an MD Acumen observership count toward CREST?

An MD Acumen UK Primary Care Observership is a structured clinical observership programme that provides documented clinical exposure, reflective learning, and reference letters. It is not itself a Trust-Grade post and does not on its own satisfy CREST sign-off, but it strengthens an IMG's pre-application file and orientation to the UK clinical environment ahead of Trust-Grade post applications.
